Order forms
Once you have selected the resources you wish to borrow or request you need to complete an order form and send it to the health improvement information and resources service. The process for ordering and delivery is described fully in the Resources page of this site.
Once you have completed the form, you can fax it to us on: 01670 816796. Alternatively you can post it to us at the following address:
