Community rehabilitation teams
The community rehabilitation teams in Northumberland offer a specialist service giving adults a time-limited period of rehabilitation in their own homes. The service helps people recover skills and confidence and is centred around helping people achieve the goals that are important to them.
The service is provided by a team of professionals which may include:
- a physiotherapist
- an occupational therapist
- a speech and language therapist
- rehabilitation workers
- technical instructors
Once you have been referred onto the service a full assessment will be undertaken to determine your rehabiltation needs and to see what you can currently do for yourself. We will then develop an individualised rehabilitation programme. The programme will establish what achievements are most important for you and will set out goals to help you meet them. The community rehabilitation team aims to provide a quality service within your own home. We will communicate with all those involved in your care, including your family, carers and other professionals and will provide advice and support where needed. We'll also regularly review your needs and goals to ensure you are getting the most out of the service.
Currently the service is only available during the week but if support is required at the weekend, links to other services, such as home care, START or the rapid response team, can be made.
How can I access the service?
Patients can be referred into the service by any health or social care professional, including your GP, a nurse or your care manager.

There are four community rehabilitation teams across Northumberland, each one managed by a community rehabilitation team manager. The teams are available Monday to Friday, 9.00am - 5.00pm.
