Palliative care at home (PaCH)

The palliative care at home (PaCH) service provides personal care and practical support to people, carers and their families living with life limiting illnesses.

What does the service offer?

The palliative care at home service offers a support system to enable individuals, carers and their families to live as active a life as possible. Our support workers can carry out non-nursing duties and provide social assistance to patients and their families. Support workers who work for the service have a wide range of personal experiences and skills and are well supported by the palliative care at home co-ordinator, your district nurse and your GP.

Support workers can:

  • provide help during the day and /or night for people who need support in order to be able to stay at home
  • stay with patients in their own home during the day or overnight to allow relatives time to rest

What will happen?

The PaCH support worker will contact you for an introductory chat before visiting your home. In your home a comfortable place to sit and access to the toilet and washing facilities will be required. Hours of work will be agreed between your district nurse and the PaCH service co-ordinator to ensure that the best possible assistance is given to you and your family.

How do I access the PaCH service?

Your district nurse or Macmillan nurse can arrange for you to have support from the palliative care at home service. They will discuss with you what kind of assistance you need and when it will be of most use to you.
